Documents for official verification

USAC may ask for documents to verify an application. Which documents you need depends on what could not be confirmed and how you qualify. Use the official supporting-documents page for the full current list.

Program participation

A benefit award letter, statement of benefits, or other accepted record may be used to show participation in a qualifying program. USAC’s guidance explains the required name, program information, issuing organization, and valid dates.

If you qualify through a child or dependent, read the official instructions for that situation. Do not send this website a benefit letter, screenshot, case number, or dependent’s details.

Income documents

Accepted examples can include a tax return, current annual income statement, or qualifying pay records. The official guidance explains which issue dates and time periods are accepted. A single document is not automatically sufficient for every application.

Identity, address, or household questions

An official application may require information or documents to confirm identity, residence, or household status. Read the request you receive and submit only through the official process or an authorized participating provider.

Protect your records

This site does not take applications and has no document-upload service. Our checker does not need your name, birth date, Social Security number, benefit case number, home address, or exact income.

Before sharing records elsewhere, check the destination carefully. The official links below do not include any of your checker answers. The site you visit has its own privacy practices.

If a request is unclear

Use the Lifeline Support Center for application questions and your service company for account or billing questions. Do not send sensitive records to an unrelated information website.

Independent information

This guide does not decide eligibility or take applications. Requirements can change. Confirm current details with the official program.
